In a small saucepan, whisk together the unsweetened cocoa powder, sugar, salt, and 2 tablespoons of ![hot water] over ^[medium-low heat]. Stir constantly for about 1 minute until the mixture forms a smooth, glossy paste. This step "blooms" the cocoa powder and prevents lumps.

Pour in the milk (and heavy cream if using), whisking continuously to fully incorporate the cocoa powder paste into the liquid. Increase heat to ^[medium].
