In a small saucepan, whisk together the 2 tbsp polvo de cacao sin azúcar natural, 2 tbsp azúcar granulada, 1 pinch sal, and 2 tablespoons of hot water over medium-low heat. Stir constantly for about 1 minute until the mixture forms a smooth, glossy paste. This step "blooms" the polvo de cacao o canela para espolvorear and prevents lumps.

Pour in the 2 cups leche entera (and ¼ cup crema pesada if using), whisking continuously to fully incorporate the polvo de cacao o canela para espolvorear paste into the liquid. Increase heat to medium.

Heat the mixture, stirring frequently, until it is steaming and just beginning to simmer around the edges — about 5 to 6 minutes. Do not let it come to a full boil, which can cause the 2 cups leche entera to scorch or develop a skin.

If using chopped chocolate semi-dulce, remove the saucepan from the heat and add it now. Whisk until completely melted and the polvo de cacao o canela para espolvorear is velvety smooth.

Stir in the ½ tsp extracto de vainilla puro. Taste and adjust sweetness — add another teaspoon of azúcar granulada if you prefer it sweeter.
